The Lions advance to a Thursday semifinal vs. the winner of the San Jose Earthquakes-Minnesota United FC quarterfinal that will be played Saturday.
LAFC's Jordan Harvey hit the crossbar in the second round of the shootout, paving the way for Nani to clinch the victory in the bottom of the fifth frame. Nani got revenge on Kenneth Vermeer, who stopped his penalty kick in the 57th minute.
A goal by Bradley Wright-Phillips in the 60th minutes put LAFC in position for the win until Joao Moutinho's header off a corner kick by Nani tied it 1-1 in the 90th minute.
Moutinho was the No. 1 pick of the 2018 SuperDraft by LAFC but was traded to Orlando after one season.
Wright-Phillips, who had 108 goals the previous seven season for the New York Red Bulls, scored his fourth of the tournament on LAFC's first shot of the game to cap a swift change in momentum at the ESPN Wide World of Sports Complex.
Three minutes earlier, Vermeer kept the match scoreless after LAFC defender Diego Palacios fouled Ruan on the right side of the penalty area.
Vermeer read Nani's penalty shot correctly, went to his right and made a two-handed stop.
LAFC took advantage, and Diego Rossi, who leads the tournament with seven goals, earned his third assist, setting up Wright-Phillips.
Rossi dribbled to the end line then sent the ball to the middle of the box where Wright-Phillips was able to get his left foot on the pass before shooting.
It was the second time Wright-Phillips had put the ball in the net.
In the 32nd minute, his goal was taken off the board after a video review because Brian Rodriguez played the ball to him from an offside position.
